If you have lost or misplaced a certificate to a class, you will need a replacement before you attempt to renew your license. There is a charge for a lost certificate as follows:

• $10 for a live class you attended if you provide us with the specific class and date attended.
• $20 for a live class you attended if you do not provide the class or the date attended.
